It seems the length would be relative to the shoe worn at that measuring, and that could change.

That's true in a sense. If you plan on wearing heels with all of your dress slacks, you would wear a standard size heel to the fitting and wear those with your slacks. If you are having jeans altered and tend to wear sneakers of flats, you would wear those shoes to the fitting.

INSTRUCTIONS: Place the trouser marker over the front of the shoe,resting against the shin vertically using a pin or chalk, make a mark on each pant leg. this reference mark will be 12" from the floor every time. Rest the Trouser Marker against the calf vertically. make a mark across the top of the trouser marker on each pant leg this reference mark will be 12" from the floor every time. Pin the front and back of the pants to the desired length. (remember you can also use the trouser marker to mark the in or out seams.) The measurement between your chalk mark and the desired pinned length is the established measurement for that specific customers' hem. The established length will remain the same for each pair of fitted slacks.
